A recent Florida enforcement case is a sharp reminder that fall protection cannot be treated as optional paperwork.
In a news release issued on July 23, 2026, the U.S. Department of Labor said OSHA cited Orchids Builders LLC after inspections at two residential worksites in Rockledge, Florida. Roofing work on low-slope roofs has additional permitted methods and conditions under the standard. Steep-roof work has its own requirements. Because roof configuration and task matter, contractors should not rely on a single generic plan for every project.

For personal fall-arrest systems, verify suitable anchorage, connectors, harness condition, clearance and rescue planning. Prevent swing-fall exposure and do not assume an attachment point is rated simply because it appears strong.
For guardrails, warning lines or other systems permitted in a particular roofing situation, follow the exact criteria in the OSHA standard. When conditions change, reassess the plan.
Treat Ladders as Part of the Fall Plan
Roof access is one of the most repeated exposure points. Extension ladders used to access an upper landing surface generally must extend at least three feet above the landing, or be secured and provide a grasping device when that extension is not possible under the rule.
Set ladders on stable surfaces, secure them against displacement, maintain the proper angle and keep the access area clear. Workers should face the ladder and maintain safe climbing practices. Inspect ladders and remove defective units from service.
A strong roof-edge system does not protect a worker who falls while getting on or off the roof.
Make Training Verifiable
Training should be understandable to the employee and cover the hazards they will actually face. Workers need to know how to erect, maintain, inspect and use the selected systems, as applicable to their roles.
Document the employee name, training date, subject, instructor and method of evaluation. Retrain when workplace changes, equipment changes or performance shows that knowledge has not been retained.
A signature alone is not a safety culture. Supervisors should observe work, correct deviations and record toolbox talks and inspections.
